What is a Life Care Plan?
A life care plan is a comprehensive, patient-centered document used to project future medical, psychological, rehabilitative, and supportive care needs for clients with life-changing injuries and/or chronic medical conditions. It is based on an extensive analysis of the client’s medical records, in-depth interviews with the client and their family, collaboration with medical experts, and review of evidence-based practices. Using data-driven and evidence-based research, the life care plan quantifies the costs needed to address the client’s medical care throughout an estimated life span. The goal of the life care plan is to restore the client and their family to a level of functioning before the catastrophic injury and/or chronic medical condition, address quality of life, restore independent functioning and lifestyle, and prevent secondary diagnoses and complications throughout the client’s life.
The in-depth process of life care planning involves a structured approach to assess, diagnose, identify outcomes, plan, implement, and evaluate the healthcare needs of each client. Meyers Nurse Consulting utilizes a systematic methodology to create individualized life care plans:
- Referral & Case Review – Understand the client’s needs and gather pertinent medical records.
- Comprehensive Assessment – Evaluate medical, functional, and psychological needs via in-person or remote interviews with the client and their family.
- Care Needs Identification – Collaborate with medical experts to evaluate the client’s health status and future medical needs.
- Plan Development – Outline future medical care, therapies, and supportive services with careful consideration of the client’s goals, needs, and interests.
- Cost Analysis – Estimate lifetime costs for treatment, equipment, and care utilizing usual customary, and reasonable (UCR) charges.
- Implementation & Updates – Recommendations for plan execution and review the effectiveness of the plan, making revisions as needed.
